This is a convenient way of bringing an image into Image Optimizer from another application e.g. a paint program.
Most applications will let you copy an image to the clipboard. Often it is a simple one button click.
Copy the image to the clipboard in the application you are using. Switch to Image Optimizer and press the Paste button 
or Edit->Paste from the menu.
